Left and right arrows to accelerate or to move to left or right and Z or Shift to jump. With K you kill yourself (if you are blocked) and P is for Pause.

You have a limited life for a time that is shown in a bar above. The life should be enough for you to finish the 5 levels from an episode. You can pick up the colored points to prolong your life. If you die, the level of life is canceled.
